Note that there are two versions of the chain of custody form. One is designed specifically for air samples for TO-15 analysis and another for all other matrices being analyzed. Each chain of custody form has two formats a static printable version and interactive version that allows you to fill out most fields in advance of sample collection.
The static blank PDF copy is available if you want to print out and complete by hand. With the editable PDF document, you can fill out most fields of this interactive chain of custody form on line and print it out before going in the field. With this version, field paperwork can be reduced to only sampling event specific information (sample date and time, signature, etc.). The blank interactive form can be downloaded to your computer and used as a template any time. If you like, save the form you edited as a PDF to use each time for repeated monitoring events.
